For any doctor to be employed in the NHS and for MediQuest to represent you; the below documents are a pre-requisite. We can assist you in obtaining any of the documents below within reason;

 

Personal Documents

 

  • An up to date CV (either electronic or hard copies)
  • References X 2 (one from your most recent employer and within the last 12 months)
  • Copy of your Passport with relevant stamps of right to work in the UK.
  • Two Passport Photo’s for our files and ID Badge
  • GMC Certificate
  • CRB Check certificate
  • Osmosis Training Certificate
  • Qualification Certificate

 

Occupational Health Documents

 

  • Proof of immunity for Hep B (laboratory report (blood test) of Titre level)
  • Proof of immunity for Hep BsAg (laboratory report (blood test) of Titre level)
  • Proof of immunity for Hep C (laboratory report (blood test) of Titre level) (EPP Only)
  • Proof of immunity for MMR (laboratory report (blood test) of positive immunity)
  • Proof of Tuberculosis immunity (written proof that BCG scar been viewed by an OH dept, nurse or doctor)
  • Proof of immunity for Varicella (self declaration of having Chicken Pox or Shingles)
  • Proof of immunity for HIV (EPP Only)
  • Proof of immunity Measles and Mumps
